They were having a lovely interview when Oprah asked "does Burt sleep in his toupee on?" referring to Sally's then relationship with Burt Reynolds. Producers had told Oprah she had to ask as the audience would want to know but as soon as she did, Sally turned cold on her, shut down and the TV host couldn't get in again. Sally and Burt were in a relationship between 1976 and 1980.
It was live TV so there was no opportunity to edit the awful question out. Oprah said she still cringes thinking back about asking the question.
